Injuring a Survivor will call upon The Entity to block pallets from being pulled down within a 32 meter radius of the Survivor location for 15 seconds.

The Hex effects persist as long as the related Hex Totem is standing.

At the start of the Trial, 4 random Hooks are changed into Scourge Hooks:

The Auras of Scourge Hooks are revealed to you in white.
Each time a Survivor is hooked on a Scourge Hook, the following effects apply:

The Generator with the most Progression explodes, instantly losing 9/12/15 percent of its Progression and starting to regress.
Survivors repairing that Generator will scream, but not reveal their location.

Any Survivor repairing a generator suffers from the Blindness and Exhausted status effect.  If the Survivor is already Exhausted, their status effects timer will be paused.

Both status effects linger for 5 seconds after stopping repairs.

Leverage

Each time you hook a Survivor, Leverage activates and gains 1 Token, up to a maximum of 10 Tokens:

For each Token, the Altruistic and Personal Healing speeds of Survivors are reduced by a stack-able 3/4/5 percent for the next 30 seconds, up to a maximum of 30/40/50 percent.

The Auras of Survivors within 2/3/4 meters of any Dull Totem are revealed to you.

When another Perks Hex Totem is cleansed or blessed, its Hex is transferred to the Hex: Undying Totem, including any accumulated Tokens, disabling Hex: Undying instead.
The Hex effects persist until its Hex Totem is cleansed.

Your basic attacks make Survivors drop their item on impact.  While on the ground, it takes up to 90 seconds for the item to lose all of its charges if it is not picked up.  Reveal items on the ground within 32 meters with a white aura.  The aura will slowly fade to red until the item is empty of charges.
